Job Title: Field Nurse Supervisor

Reports to: Clinical Director


Use principles of supervision, reflective practice and staff development to provide primary support and appropriate oversight to Field Nurses and to build and model a strengths-based, client-centered culture.


¬∑ Develops Initial Plan of Care (also referred to as Plan of Treatment) and ensure it is properly and closely followed by other home health employees. This will be the result of successfully assessing the condition of patients‚ health and places of residence, thoroughly reading any Doctor‚s orders and checking the place of residence for any issues with accessibility.

· Sets up and maintains clinical records for each patient visited.

¬∑ Makes follow-up visits to review patients‚ progress, ensure Field Nurses are providing quality and effective health care and make necessary adjustments and recommendations in areas that may be lacking. These visits will require the administration of services that can only be provided by a Registered Nurse. These visits may also include visits to patients who have expressed dissatisfaction with their care or request follow-up clinical services not included in the original Plan of Care.

· Assists Field Nurses with risk assessments and investigations of injury.

· Ensures places of residence are fully and properly equipped with required supplies and equipment.

· Educates patients and their families about the condition of their relatives while delivering exceptional customer service.

¬∑ Conducts pre-assessment and post ‚ hospitalization visits.

· Works directly with the Clinical Director to ensure the best allocation of Field Nurses.

· Ensures that Field Nurses perform required duties in accordance with company policies, rules and regulations.

· Provides assistance to Field Nurses with crises and medical management emergencies.

· Assists Clinical Director with education and professional development of Licensed Practical Nurses and Registered Nurses via the Training with Care Orientation to close necessary knowledge and skill gaps.


· Graduate of an accredited School of Practical Nursing with a valid and current New York State License.

· Must be a registrant or a Registered Nurse Pursuant to Article 139 of the New York State Education Law.


· Minimum of two years in Community Health, Program Management or a Home Health Agency as a Registered Nurse.

· Ability to effectively communicate verbally and in written formats at all levels.

· High level of professionalism with ability to handle sensitive and confidential information.

· Meticulous attention to detail and strong organizational skills.

· Ability to interact with and relate to the problems and difficulty associated with the aging process.

· Ability to provide direct care tasks in accordance with the established patient Plan of Care.

· Must work cooperatively with others, and perform a wide variety of complex and complete tasks involving treatments and procedures.
